Our Services

Hospice

When patients are faced with a life-limiting illness, the focus shifts from curing to caring through hospice, also known as end-of-life or comfort care. When a physician determines a patient may have a life expectancy of six months or less, trained hospice nurses provide a specialized type of medical care focused on symptom management and quality of life. In addition to physical, emotional, and spiritual support, Lifesong’s complementary therapies such as music therapy and/or pet therapy may also be included in the care plan. Patients are celebrated through the My Legacy Program and compassionate and dedicated volunteers provide support to patients and their families throughout their journey.

Care process

Within the first five days, the care team including a physician, nurses, a social worker, a chaplain and a hospice aid are assigned to provide care aligned with the personalized care plan discussed during the assessment. This allows family and friends to enjoy quality time with their loved one.

Families receive timely, compassionate messages with guidance, resources, and check-ins in person, on the phone or via text—from the moment of admission to 13 months of bereavement. Whether it’s making arrangements, finding local support, or just navigating what comes next, we connect families to vetted tools and compassionate experts. We’re here for you.

Lifesong is here to support every step of your hospice journey

Palliative care

Palliative care is a specialized form of medical care that focuses on relieving symptoms, providing comfort, and supporting people living with serious illnesses. At its core, palliative care is about improving quality of life for both patients and their families.

Palliative care is an additional layer of support that works alongside the care you already receive from your medical team. It centers on your personal goals, values, and priorities, helping you live as well as possible.

You can receive palliative care while continuing treatments, including therapies such as physical or occupational therapy. It does not replace your current medical team — it enhances it.

(At this time, Lifesong’s palliative care services are only offered in specific areas in Central Pa.)
